1. Introduction

At Psych2Gether, we provide personalised, evidence‑based mental health support including assessments, therapy, and psychiatric care for conditions such as Depression, Anxiety, OCD, PTSD, Bipolar Disorder, ADHD and other mental health challenges.

Where clinically indicated, medication may form part of an individual’s treatment plan, complementing psychological and therapeutic interventions to maximise patient outcomes and wellbeing.


2. Purpose

The purpose of this policy is to ensure safe, ethical, evidence‑based practices in prescribing, reviewing, and managing psychotropic medications for clients under Psych2Gether care — including clear expectations, informed consent, monitoring, and collaboration with other health professionals where appropriate.


3. Scope

This policy applies to all clinical staff, including consultant psychiatrists, psychiatric prescribers, and allied care professionals engaged in medication management on behalf of Psych2Gether.

It encompasses:

  • Initiation and review of medication

  • Documentation and consent processes

  • Communication with clients and external healthcare providers

  • Ongoing monitoring and risk management


4. Principles of Medication Management

Psych2Gether is committed to the following standards:

4.1 Patient‑Centered Care
• Decisions about medication are always grounded in best available evidence and tailored to the individual’s needs and preferences.
• Clinicians discuss benefits, risks, alternatives, potential side effects, and expected outcomes in a transparent way before initiation.

4.2 Informed Consent
• Clients must provide explicit informed consent before any medication is prescribed.
• Clinicians ensure understanding of the plan, including instructions for use, monitoring requirements, and indications for review or discontinuation.

4.3 Collaborative Care
• With client consent, Psych2Gether clinicians may communicate prescribing plans and changes to the individual’s GP or primary care clinician to support continuity of care.

4.4 Evidence‑Based Prescribing
• Prescriptions follow national clinical guidelines and recognised best practices, accounting for diagnosis, history, comorbidities, and treatment goals.
• Off‑label use (when clinically justified) is discussed with clients and documented.


5. Assessment and Prescription Process

5.1 Clinical Assessment
• Medication decisions are based on a comprehensive clinical assessment including symptom history, diagnostic formulation, previous medication exposure, adverse reactions and physical health considerations.

5.2 Initiation and Review
• A clear follow‑up and review schedule is established at the outset, aligned with clinical need and safe practice (e.g., weekly to quarterly depending on stability and risk factors).


6. Monitoring and Safety

6.1 Monitoring Practice
• Regular monitoring requires tracking clinical response, side effects, compliance, and any risks.
• Clients are advised on how to report concerns promptly (e.g., emergent side effects, mood changes, or escalation in symptoms).

6.2 Risk and Crisis
• In case of acute risk (e.g., adverse reactions or crisis), clients are instructed to seek urgent medical care or contact local emergency services.


7. Records, Documentation & Confidentiality

• All medication decisions, consents, discussions and reviews are documented securely in the client’s clinical record.
• Psych2Gether complies with applicable data protection and privacy standards in handling clinical information.


8. Responsibilities

Prescribers are responsible for clinically appropriate prescribing and oversight.
All staff ensure consistency with this policy and escalate concerns regarding medication safety or quality.


9. Policy Review & Quality Assurance

• This policy is reviewed at least annually by senior clinical leadership to ensure alignment with evolving standards and regulatory requirements.
